4* Have you had your blood pressure checked within the last six months?

5* Is your blood pressure very high, a little high, normal, a little low, or very low? (Answered by those who had their blood pressure checked)

6* Have you had your cholesterol checked within the last six months?

7* Is your cholesterol very high, a little high, normal, a little low, or very low? (Answered by those who had their cholesterol checked)

8* Have you seen a doctor for a general physical exam in the last 12 months?

9* After that physical exam, did your doctor recommend any lifestyle changes in the way you eat, drink, or exercise? (Answered by those who had a physical)
